Footwear Modifications
48yo male with traumatic injury including fractures of a vertebrae, pelvis, left femur, and left tibia and fibula, sustained after falling from an 18-storey building. Malunion of compound left tibia and fibula fracture resulting in a leg length discrepancy of 16.5cm. Left foot sits in a 45° fixed ankle equinus position due to muscle paralysis, and there is a left coxa valga deformity due to the way the pelvis and hip bones have healed causing a left gena varum. Our client first attended our clinic wheelchair-bound in 2016, and was scheduled in for an above-knee amputation due to his inability to weight-bear and mobilise.
Client was fitted with a pair of custom-made orthopaedic boots produced on a 3D-foot last. The left boot features a 16.5cm raise split internally under the orthosis and externally to the sole to improve aesthetics of the device. A rigid hindfoot brace is integrated in the left leather upper to support and stabilise weakened foot and ankle, a lateral sole flare to control degree of oversupination, and a high toe spring to provide greater clearance at toe-off. A pair of custom-moulded accommodative foot orthoses with deep heel cup to provide calcaneal control, forefoot valgus post to assist with propulsion, and poron offloading at metatarsal heads to reduce forefoot loading caused by ankle equinus deformity.
For the first time since sustaining his injury, the client was able to weight bear with the use of the custom footwear and orthoses solution. He spent several months in physical rehabilitation with a physiotherapist to increase his lower limb muscular strength and mobility, and relearn how to walk with his post-injury lower limb biomechanics. Over the years, we have supplied multiple pairs of footwear and orthoses with variations in technical specifications to adapt to the change in lower limb function and gait. He has transitioned from being wheelchair-bound full-time, to mobilising a few steps with a 4-wheel walker, and now progressed to independent-walking for short distances, and assisted-walking with an X-crutch for moderate distances and 4-wheel walker for long distances.
